(1) is it impolite to disallow smokers to take an arbitrary break that is not scheduled by the poker timer? since these are a group of friends, we normally concede to their request, but this really slows the game play down. Your thoughts?

I think I would ask what your local casino might do about this. They would put a marker on the players spot. They get blinded or cards are dealt and if they don't return in time…the cards go in the muck.
I am firm on this one. Even though it might seem like a casino rule, you have to keep the game going. I want my players to come back.

i have 18 smokers and two non smokers, but with the respect of the non smokers everyone smokes outside, and there is a 10 min break.

We do what Eric does. But we will also have a break here and there for bathroom, smoking or what ever you need to do.

yeah – if they go outside – the cards will be mucked. it goes the same with others who needed to go to the bathroom. I always go to the bathroom when it's my big blind because I will have time.

out of 9 players I have 6 that are smokers. no smoking in my house, so there's a 10min break for them to go out and kill some lung cells.

The 10 minute break after an hour is always agreeable , those who require further 'time out' do so with blinds posted is a fair thing to do.
